Why Does AI Hallucinate?

AI told you something that sounded completely right and turned out to be completely wrong. Here's why that happens and what you can actually do about it.

“Hallucination” is a strange word to use for a software behavior, but it stuck because it captures something real: AI doesn’t make things up randomly but generates responses that look and sound correct, with confident tone and plausible structure, that happen to be factually wrong, with no indication that anything is off.

Understanding why that happens changes how you design processes around AI.

What’s actually happening

A large language model generates text by predicting what should come next based on patterns it learned during training. It doesn’t look things up, it doesn’t check a database, and it has no fact-checking layer. When you ask it a question, it produces the most statistically likely response given everything it’s seen.

So when the most likely-sounding answer is wrong, the model produces that wrong answer with complete confidence, because it’s not lying and it doesn’t have the ability to lie. It’s doing exactly what it was built to do, just with a result that doesn’t match reality.

Why it matters more for some use cases than others

If someone uses an LLM to draft a marketing email and the tone is slightly off, the cost is low. A human reviews it, catches the issue, and fixes it. But if someone uses an LLM to pull together legal precedents, medical information, or financial figures and nobody checks the output, the cost is much higher.

The risk isn’t evenly distributed. It concentrates in exactly the places where the output sounds most authoritative, because confident-sounding language is what LLMs are trained to produce.

What you can actually do about it

You can’t eliminate hallucinations from an LLM. What you can do is design around them.

Ask the model to cite sources and verify them yourself. Use AI for tasks where a human is already reviewing the output anyway. Don’t deploy AI in high-stakes situations without a review step before anything goes out. And make sure the people using these tools in your organization understand that confident output is not the same as correct output.

That last part is harder than it sounds, because the output often looks exactly right.
